Description
Advances in medicine have vastly expanded drug treatment options, but complex medication regimens, dosing issues, side-effects, and other factors can get in the way of medication adherence, and, for many, progress in managing their disease. Pharmacists integrated into the care team can provide medication therapy management, consultations, and other supports directly to patients. These high value activities often are not reimbursable. Many 340B hospitals are using their 340B savings to fill this gap with remarkable results for patients. This session will feature three of these initiatives.
Learning Objectives:
- Explain how 340B organizations are using 340B savings to fund greater pharmacist engagement in direct patient care
- Identify specific issues patients encounter that impede proper use of medications and how pharmacists can address them
- Describe patient outcomes that can be improved through pharmacist involvement on the care team
- Describe different functions pharmacy staff can play on the care team to increase medication access
