Description
Much of the dialogue about Medication Therapy Management (MTM) is limited to a process of education and medication reconciliation with those patients for whom there is a source of 3rd party reimbursement. As a result, the two most common barriers to large scale implementation of MTM services are the value of clinical pharmacist time and the lack of direct reimbursement for the services. This session will suggest broadening the MTM dialogue to encompass the impact clinical pharmacy services have on quality measures, cost effectiveness, and population health outcomes. Set against the backdrop of emerging models of value-based reimbursement, participants will learn about the innovative clinical programs at three health centers and the measurable value they are returning to the patients and communities served. Participants will learn about a variety of programs: one of these health centers operates entity-owned pharmacies, another provides prescription medication solely through contract pharmacy partners, and the third isa Ryan White grantee serving multiple states.
